You will have the choice of scheduled limited medical benefits or major medical benefits. Scheduled Limited Medical Benefits: C1 Healthcare Solutions provides scheduled, first dollar benefits that meet some of the most frequently used healthcare services of the working family. Scheduled limited medical benefits are only the first component of this program. This program is also based on the largest PPO network (Beech Street) and also includes a pharmacy plan, accidental medical, AD&D and a health savings card. Major Medical Benefits: Extend Benefits will bring you three to five insurance companies and 75 - 125 plan options from which you  can choose. These major medical vendors include:
Premiums vary depending upon the features of the plan, such as: deductible, coinsurance, prescription drug coverage, etc. These programs all have underwriting criteria.

Additional Pay & Perks:

Longevity bonus: contractors who stay continuously leased on with Swift receive a progressive bonus per mile after the first three years of service.

Base plate licensing and permits are provided.

$35 for each additional drop/pick on multi-stop loads (after the first).

100% of lumper's fee, if required, are paid.

Contractors are insured while under dispatch by Swift Transportation.

Tolls are reimbursed.

Swift Terminal Locations:
Drivers can be based at any of the 34 full-service Swift terminals, your home away from home. Terminals are your source for fuel, maintenance and repair service. Fuel lane personnel are trained to assist drivers with safety and maintenance checks.

Owner Operator Requirements:
-
Must be 21 years of age
-
The first and second seat drivers must have at least eight months of recent, verifiable OTR experience
-
Must be professional and have a clean and neat appearance
-
Must be able to pass a DOT physical and drug screen
-
Must possess a valid Class A CDL with Haz-Mat endorsements
-
No DWI or DUI convictions within the past 5 years
-
No preventable accidents within the past 12 months; no more than 2 in the past 5 years
-
No reckless driving convictions in the last year and NEVER in a commercial vehicle
